Requirements for obtaining NOC for Kidney/Liver Transplantation:

1. 01 (one) copy of duly filled in application form (download)

2. Request Letter from the Hospital

3. Declaration form (download) from the Hospital

4. Affidavit from Patient, Donor and their guardians (Must be attested/e-apostille by the Ministry of Foreign Affairs in Dhaka)

5. If the Guardians of the Donor/Recipient (if unmarried, then Parents)/ (if married, then Spouse) is dead and Donor/Recipient wants to use another person as Guardian, then death certificates need to be submitted (Must be attested/e-apostille by the Ministry of Foreign Affairs in Dhaka)

6. Family Tree/Certificate of the relationship between the donor and the patient (Must be attested/e-apostille by the Ministry of Foreign Affairs in Dhaka)

7. In case of a second blood relation between donor and recipient, the donor's guardian should be a first blood relative of the donor.

8. Documentation/affidavits involving the donor needs to be done in his/her own district. Similarly, recipient’s documentation/affidavits needs to be done in his/her own district,

9. Passport & Visa copies of the patient, donor and guardians (Must be attested/e-apostille by the Ministry of Foreign Affairs in Dhaka)

10. Donor’s Declaration in Person at the Mission (the donor needs to come in person in the Embassy)

11. Doctor’s Certificate outlining the need for transplantation and fitness of the patient and Donor

12. Filled in up Form-21 (download) (The Mission will endorse only the documents attested/e-apostille by Ministry of Foreign Affairs in Dhaka)

13. Fees: 25 USD for urgent issuance and 15 USD for regular issuance of NOC. The fee needs to be paid in favor of Mission’s bank account or by card (no cash accepted). The fees can also be paid in NPR equivalent to USD charge at per the rate of the day of deposit.
